After you have enrolled in Works with WELL, it's time to complete your application and submit for review. Keep reading to learn how to complete these documents and submit your application.

Product List

The product list contains the full inventory of SKUs you want to earn the licensing mark for. The list gathers basic project details and will be used in your project’s directory page upon review.

1. Fill in the top section of the form including product group category, type, description and URL. This will be used in your Works with WELL profile and will link to your organization’s web page.
2. Complete all fields marked as “required” in the column header (columns A-L). Columns M-O are optional.

Application form

Application form templates must be used for each product group you submit for review. This sheet indicates which WELL Standard requirements your product aligns with. Save and upload separate sheets for each project group that shares the same feature alignment and supporting documents 

1. On the SKUs tab, enter your product group’s name and all applicable SKUs.
2. On the Questionnaire tab, select the product category and product type. Make sure to select the same product category and product type that you chose on the Product List.
   • This will populate the corresponding CSI number and a set of questions you will need to answer. The questionnaire will indicate which features are mandatory and which are optional.
3. If you are proposing a new product type or feature, use the New Products Feature Proposal tab. Enter the required details and tell us how your product supports the targeted WELL features.

Upload documentation and submit for review

After you have completed the product information list and application form:

1. Return to your WELL account.
2. Click your initial/profile image in the top right and select My Membership.
3. Open the Licensing tab.
4. Answer the questions about your product category, type and aligned WELL parts.
5. Give your product group a name.
6. Upload your product list, application form and supporting documents.
