How to upload ongoing documentation
The process for uploading ongoing documentation is slightly different depending on the method. See the instructions below to learn how to upload ongoing maintenance reports or ongoing data reports.
You must upload ongoing documentation from the same scorecard you used to upload the initial documentation to achieve the feature–e.g., if you uploaded the documentation for initial achievement using the individual location scorecard, you must do the same for ongoing documentation (not the WELL at scale scorecard).
- Ongoing maintenance reports are an audited verification method. To upload your annual maintenance reports, follow the instructions below.
- Open your WELL at scale account and go to the Scorecard tab.
- Find the feature you would like to upload documents for and open the Verification section for that feature.
Click the text next to the upload button to see which locations require audit documentation.
When you’re ready to upload, click the Upload button and follow the standard document upload process.

Like ongoing maintenance reports, ongoing data reports are also an audited verification method; however, the process is different. For ongoing data reports:
- Select your own audit locations for ongoing data reports. The number of required audit locations is equal to the square root of the number of locations that have achieved the feature, rounded down. You do not need to indicate which locations were chosen on your Scorecard. Use the same locations each year, adding locations as necessary when additional locations achieve the feature.
- Prepare documentation showing that you meet the requirements at each of your chosen audit locations.
- Upload your audit documents to all locations that have achieved the feature, clearly indicating within the document the name and ID of the locations you have chosen as audit locations.

This organization has achieved A01.5, which requires ongoing data reports, at 7 locations. This means 2 locations must provide audit documentation (√7=2.65, rounded down to 2). You may choose any 2 locations from the 7 that have achieved the feature to provide audit documentation.
Click the text indicating how many locations achieved the feature to see the list of locations that have achieved the feature.
In this example, we will choose to prepare documentation for Example location 1 and Example location 2. We will then upload the completed documents to all 7 locations.

No, in years where recertification performance verification is conducted, projects can use performance verification results to satisfy both performance testing and certain ongoing testing requirements.
Projects can use performance test results as ongoing annual tests. For features that require quarterly or semi-annual testing, performance test results can be applied to the quarter or half in which testing occurs. Projects should include a copy of any performance test results within the required ongoing data report.
This guidance also applies to projects approved to collect reduced sampling for recertification.
